There are over 15 new unique items to be found from within the Forbidden Sanctum and throughout Wraeclast and the Atlas. It includes 1 designed by a supporter and 3 designed by winners of Boss Kill Events. GGG has also massively increased the power and rarity of ten existing endgame unique weapons to bring them in line with some of Path of Exile's most iconic unique items.

In this expansion, added 14 new unique items including a league-exclusive ring, a new pinnacle boss reward, a supporter unique, six new Grand Spectrum jewels and even some levelling uniques. Also improved over 100 unique items, including both levelling uniques and endgame ones. Finding a unique item is now more likely to be an exciting moment, especially during the campaign.
Introduced another unique map, The Tower of Ordeals. In this map, you may challenge the Trialmaster and potentially earn unique items from the Ultimatum league. You can create this map by completing endgame Vaal content.
Reduce the number of unique items dropped throughout the game. Rebalance a large number of unique items to make them much more distinctive and exciting, at least for the level at which you can first obtain them. This set of changes focuses on unique items available during the campaign, but we will consider changes to higher level unique items for future expansions.
Unless specifically stated otherwise, a Divine Orb should be able to be used on existing versions of these unique to obtain the new values of modifiers, but cannot be used to gain new modifiers.

PoE 3.19 introduced extremely rare foil versions of Pinnacle Unique Items. These are functionally equivalent to the regular versions, just shiny.

In this expansion it's possible to very rarely find special foil versions of any pinnacle boss unique item. These have the same stats as regular ones, but are certain to be highly prized by collectors or wealthy players wanting to show off.
The original plan was to develop a new method of making fated unique items to properly replace Prophecy. However, with a second round of fated uniques merged with their base versions by the changes listed above, it became clear that there were not enough remaining fated unique items to support such a system. We are instead just disabling fated unique items from dropping.
As a result of this, the Fated Uniques Atlas Passive Cluster, and the Rare cluster opposite it, have been removed and replaced with skills that make it more likely to find specific Master Missions:
Added 6 new Unique Items, as well as new variations of 4 existing Unique Items. 5 of these new Unique Items were designed by winners of the Siege of the Atlas Boss Kill Event.

As you travel through Wraeclast, you may uncover or assemble Unique Sentinels. These valuable Sentinels behave in very specialised and powerful ways. For example, The Basilisk causes a wide area-of-effect blast that has a petrification effect on monsters. The Hollow-Eyed Skull is basically a Headhunter in Sentinel form. Rework the Blessing mechanic of March of the Legion to limit the number of Blessings you can have to one. Change the skill to cost mana instead of temporarily reserving mana. The process of making this change also highlighted a number of problems with other mechanics which has led to a ripple effect of other changes being required. Release Blessing as a Support gem, to make the mechanic available to builds that are not using the unique item. This is intended to be useful for high mana-use characters who wish to leave their mana unreserved and for characters using the Blood Magic keystone. Improve the aura level granted by March of the Legion so the item excels at using a Blessing aura. Remove Mortal Conviction and the life passives from behind the Blood Magic keystone and make a Mortal Conviction-style support gem that makes an aura free, and prevents you reserving mana. It will be mutually exclusive with Blessing skills, so you won't be able to use both simultaneously. This support gem provides slightly less aura effect to the skill than the Blessing Support but has no cost or upkeep involved. Change Blood Magic to also grant 10% more life to compensate for the removal of the efficient life passives behind the keystone. Change Supreme Ego to not limit you to one aura and instead give more aura effect based on how much mana an aura reserves.
Change Shade Form's cooldown to pause while the skill is active. Reduce the cooldown from 45 seconds to 40 seconds.
Strength of Blood's damage reduction no longer scales with modifiers to life recovery rate.
The Dancing Dervish's "Rampage" modifier now also has "Melee Hits count as Rampage Kills", allowing players to easily build up a new rampage streak without having to kill monsters. This stat will not affect you while rampaging as Dancing Dervish disables itself on rampage but the existing behaviour where the minion generates rampage kills on unique hits is still present. The minions summoned by Dancing Dervish now use the new, channelled version of Cyclone. Their movement speed and AI have also been adjusted to feel more proactive while clearing.
Perfidy no longer has "Dread Banner has 100-200% increased Fortification duration". Instead, it now has "Dread Banner grants an additional +2-4 to maximum Fortification when placing the Banner". This change affects existing items.
Void Shot (triggered by the Voidfletcher Unique Quiver): Now deals 25% more Area Damage (previously 100%). Now has 65% Effectiveness of Added Damage (previously 120%).
Introduced a new "Synthesised Map" Base Type, which all 5 Unique Synthesis Maps now use. This Map is not found on the Atlas.
Some of the Unique items that drop from The Maven are not powerful enough due to changes to the structure of the Endgame content. Improve these Uniques so they feel appropriate as a reward from an Endgame boss.
Trypanon now has "This Weapon's Critical Strike Chance is 100%" instead of "All Attacks with this Weapon are Critical Strikes". This change means that modifiers to Critical Strike Chance can reduce the weapon's critical chance below 100% now.
Unique Quivers have been reassigned to a base type that is most appropriate for them and their level:
Remove the Prophecy mechanic from the game, and move many of its rewards to other content.
The following Uniques have had their damage adjusted, to compensate for the buffs to Bow base types:

Unique items are indicated by the orange color of the name. A Unique Item(Search from Path of Exile Trade) always has a specific name, unique artwork, and a predetermined list of modifiers. Generally, unique items are meant to be less powerful overall than high-level rare items, but offer unique gameplay and allow special character builds to be based around them.
