The acquisition rate of Forgotten souls from Salvaging has been adjusted: Salvaging a Sacred item has a 5% chance to grant a Forgotten Soul.
The following loot adjustments have been implemented per World Tier. World Tier III: Only Legendary, Unique, Uber Unique, and Sacred Rare items will drop. Players will instead receive additional crafting resources.
Sacred Items are more powerful than regular items. Sacred items and Unique items can drop in World Tier 3. Ancestral items can drop in World Tier 4.
Normal item < Sacred item < Ancestral item. A made-up example:
Aside from the Common, Magic, and Rare rarities, it's also possible to find Sacred and Ancestral versions of Legendary and Unique items. These versions feature potentially stronger effects and stat rolls, making them incredibly sought-after in the late game.
Sacred items are essentially better versions of the normal, magic, rare, and legendary gear that you got in the Veteran world level, but you can also get Sacred unique gear, since unique items also unlock as potential drops in the Nightmare world tier. The key difference is that Sacred items roll higher stats than their regular counterparts, so are essentially just better versions of those same items.
Generally, I'd suggest holding onto Sacred rare gear for potential building and upgrading into legendary, while you can sell Sacred normal and magic gear, which seems to be worth a bit more than its regular version would be. The best gear you can get is Sacred legendary and Sacred unique—besides the ancestral items that can drop at world tier four, these are the most powerful available.
Nightmare Dungeons will be excellent for farming Sacred and Ancestral items, as well as upgrading Glyphs.

At the Occultist, you can imprint Legendary Aspects onto Rare Sacred and Ancestral gear, allowing you to boost their power even further.
In addition, it's also possible for Sacred and Ancestral gear to enhance the stat ranges of imprinted Aspects, making them incredible upgrades when moving up from Veteran and Nightmare difficulties.
Note that Sacred and Ancestral items aren't counted as exceptions when selling and salvaging items. To avoid accidentally disposing of valuable gear, we recommend using the Mark as Junk mechanic in the inventory and only selling marked items.

Ancestral items are the most powerful Legendary items and will replace a player’s Sacred and regular Legendary items when found (although that won’t be often). They drop when a player reaches the highest levels of the game.
