Diablo IV utilizes an open world structure with five unique regions. These are Kehjistan (specifically its northern desert region), Scosglen, the Dry Steppes, the Fractured Peaks, and the swamplands of Hawezar. Each of these areas will be 10–20 times larger than even the largest areas in Diablo III. Each of the regions are connected, and players can traverse the areas without interruption. The geography of the regions shifts in accordance with this. For instance, if a player heads south from the Fractured Peaks to Hawezar, they will see a series of waterfalls, as snowmelt feeds the swamps.
Kehjistan is a densely-populated region within Sanctuary. It is located south of the Dry Steppes and west of Hawezar. Major terrain features within Kehjistan are vast dunes in the north and rainforests in the south. Kehjistan is home to the Mage Clans, cultists and demons. Known locations within Kehjistan include the Borderlands and the capital city of Caldeum.

As we’ve seen from previous visits to Alcarnus and Caldeum, the desert shadows of Kehjistan make excellent cover for a group of rising cultists. They quietly plumb abandoned ruins, seeking power to aid the return of the ancient Prime Evils.
This arid land draws its visual inspiration from the Middle East and the Mojave desert. Once a thriving civilization, it now lies in ruins buried under sand dunes dotted with oases and teeming with threats such as giant scorpions and plenty of bandits. Kehjistan (along with Hawezar) is also home to Truine cultists, a faction previously mentioned only in lore, making its first real appearance in a Diablo game. For Diablo III fans, Blizzard says players will get to see how Caldeum, the capital city, has fared in the many years since the events of that game. While the team doesn’t want to spoil plot details surrounding such an important campaign landmark, it does tease that the situation has been less than great.
