The item power ranges for Ancestral items have been adjusted.
Level Scaling examples. Note this is iPower range for items dropped from Monsters of the listed level:
The acquisition rate of Forgotten souls from Salvaging has been adjusted. Salvaging an Ancestral item has a 10% chance to grant a Forgotten Soul.
The following loot adjustments have been implemented per World Tier. World Tier IV: Only Legendary, Unique, Uber Unique, and Ancestral Rares. Players will instead receive additional crafting resources.
Ancestral Items are the most powerful items and will replace a player’s Sacred and regular items when found (although that won’t be often). Sacred items and Unique items can drop in World Tier 3. Ancestral items can drop in World Tier 4.

Aside from the Common, Magic, and Rare rarities, it's also possible to find Sacred and Ancestral versions of Legendary and Unique items. These versions feature potentially stronger effects and stat rolls, making them incredibly sought-after in the late game.
To get Diablo 4 Ancestral items, you must play the game in World Tier 4: Torment difficulty. You can unlock this World Tier by completing the Fallen Temple Capstone Dungeon northeast of Dry Steppes in World Tier 3: Nightmare. Keep exploring dungeons in this mode, and you’ll soon come across them.
Nightmare Dungeons will be excellent for farming Sacred and Ancestral items, as well as upgrading Glyphs.
Ancestral gear can be imprinted at the cultist with Diablo 4 Aspects.
At the Occultist, you can imprint Legendary Aspects onto Rare Sacred and Ancestral gear, allowing you to boost their power even further.
In addition, it's also possible for Sacred and Ancestral gear to enhance the stat ranges of imprinted Aspects, making them incredible upgrades when moving up from Veteran and Nightmare difficulties.
Note that Sacred and Ancestral items aren't counted as exceptions when selling and salvaging items. To avoid accidentally disposing of valuable gear, we recommend using the Mark as Junk mechanic in the inventory and only selling marked items.
The suffix does not protect it from being sold as junk. Therefore, when you’re selling gear at shops or scrapping it for Diablo 4 salvage resources, ensure that you don’t accidentally get rid of it.
