As you get to the later World Tiers in Diablo 4, you’ll get access to Sacred and Ancestral items tiers, which are higher tiers of items that have more exciting stuff. There’s even a few items that are reserved for those later World Tiers.
Hell difficulty has historically followed Nightmare in Diablo games. Not much is known about the tier. Sacred and Ancestral items will begin to drop more often in this tier.
Even stronger challenges await you in the dark.

Hell difficulty requires you to complete another specific dungeon to prove you are ready. Rarer Nightmare Sigils start dropping and you receive more bonus experience. Sacred items drop more frequently, rewarding you with better loot for defeating higher level monsters. Lastly, the game continues to increase the resistance penalties.
