2. Either like Chaos Orb, reforges a rare item with new random modifiers. Or like Orb of Scouring, removes all modifiers from an item.
3. Implicit modifiers, quality, sockets, and links are not changed, as well as affixes protected by meta-crafting mods.
4. Scourged mods are retained.

In this example, it removes all of its modifiers. The original is a rare item and the new one is a normal item. As you can see, Scourged mods are retained. Implicit modifiers, sockets, and links are not removed.
