A possessed monster is very common. The color is green. It always flies in the sky. You can find and kill him easily.
There are serval main ways Possessed Monsters can appear in Path of Exile:
Tormented Spirits: This is the more common method. These are ethereal purple entities that can possess any monster in the game, granting them a buff and potentially changing their attacks.
Atlas Passive Skill: The Atlas Passive Skill "Seance" (located in the Uncharted Realms region) increases the chance for Rogue Exiles to be possessed by Tormented Spirits. This is a more targeted approach specifically for defeating possessed Rogue Exiles.

Tormented Spirits are the spirits of tormented criminals that yield their ill-gotten gains when slain. Eager to protect their trove, these spirits flee when encountered and imbue nearby monsters with dangerous powers, which improves their item drops. It's also possible for spirits to possess rare and unique enemies, greatly increasing their threat (and value) to treasure-seeking exiles.
The various spirits have differing modifiers that they themselves possess as a spirit, in addition to effects that they bestow on normal and magic monsters that they touch, as well as a powerful modifier that they grant to any rare or unique mobs that they possess (a single monster can be possessed by more than one spirit). Any mob that is bestowed or possessed by a spirit gains a modifier (Spirit's Touch/Grip) based on the spirit that bestowed them, in addition to some slight increase to movement, attack and cast speeds, as well as significantly increased IIQ and IIR. Modifiers are shown in green.
If a tormented spirit is killed, it is guaranteed to drop one rare item. If a rare monster is possessed, at least two rare items are guaranteed to drop, and if a unique monster is possessed, at least three rare items are guaranteed to drop. A Tormented Spirit that is not killed and does not find a monster to possess will eventually break its bonds and vanish, leaving no items. Spirits that do possess a monster will not vanish.

A single monster can be possessed by more than one Tormented Spirit. If a rare monster is possessed, at least two rare items are guaranteed to drop, and if a unique monster is possessed, at least three rare items are guaranteed to drop.

(2) 3+ Delirium Orb. Delirium Orbs can be applied to maps to add layers of Delirium.

Note: if Tormented Spirit dies, use Ending the Torment prophecy again and then kill the nearby rare monster.
How to achieve Defeat a Possessed Unique Delirium Boss
I recommend using two or more of them.
Keep some of Ending the Torment prophecy in inventory. Use them after encounter the boss.
Ok. I just met the boss.
Then, move to a safe place for using prophecy.
Change skills as necessary to avoid killing the Tormented Spirit.
Next, find a rare monster to complete the Ending the Torment prophecy.
If the prophecy completed, a random Tormented Spirit may be spawn on the unique Delirium Boss.
If Tormented Spirit dies, use the prophecy again and then kill the nearby rare monster.
Tormented Spirit has successfully possessed the boss!
