Duriel returns as an enemy in Diablo IV. Duriel, the Lord of Pain, is one of the four Lesser Evils, and the “twin brother” of Andariel.
His appearance is slightly altered compared to his Diablo II incarnation: he is less insect-like and more reminiscent of Ghom, with a fanged vertical maw in place of a stomach, more bestial and less insectoid jaws.
Gameplay video features him performing a fatal attack on a Barbarian, involving impaling him with a claw and devouring him with a mouth on Duriel’s torso.
By all accounts, Duriel loves to torture and inflict pain. However, he does not seem to care what the pain means to the recipient. He appears to love the music of their screams, and their agony on a sensual level. According to Deckard Cain, Duriel considers himself a master of pain but is only interested in physical torment rather than mental anguish. Many writings suggest that if Duriel were not in a position to inflict pain on a victim, he would subject himself to excruciating pain and be sated. According to Duriel himself, however, “the anticipation of pain is often more exquisite than its fruits.”
