Head into the alley on the right and soulflay the tumor that’s blocking the other two paths. Soulflay the entity that’s blocking the path on the left and head through there first. Clear out the enemies in the area and go around to the corner behind the Umbral Belly. There you can pick up the Ruq.

Once depleted, you can then stagger the enemy with a charged heavy attack, a parry, or a kick. Once staggered (red target dot) you can perform a Grievous Strike with a light attack.
