Headhunter never drops within Delve, or within a Heist.
Changes: PoE 3.14 has made a lot of changes to Path of Exile's reward systems. It has added a bunch of very powerful and valuable league-specific uniques(including Headhunter) into the core drop pool. This means they can drop from any sufficiently high-level monster, without you having to engage with specialised content. The goal with all of these changes is to make sure that every monster you kill in Path of Exile has a chance to drop something incredibly valuable, regardless of what content you're playing.
Headhunter is a Nemesis league-specific item. It can only be dropped in the maps with a Nemesis mod. The drop level is 40 and the drop chance is rare. It is one of the most expensive belts in the Path of Exile. You can search these maps from PoE Trade.
