Prime Regrading Lens is a stackable currency in PoE Heist. It changes the type of quality of a skill gem with quality to another random quality. The new gems are referred to as Alternate Quality Gems. Each existing Path of Exile skill gem has up to three Alternate Quality versions which grant different quality bonuses than the original gems. Some simply increase the power of the gem while others entirely change which builds the most benefit from the gem.

The drop level of Prime Regrading Lens is 1. It can be dropped throughout the world. However, the drop rate is rare. One of the best places to farm Prime Regrading Lens is the Grand Heist in Blueprint: Smuggler’s Den and Blueprint: Underbelly. Its Heist target: Thieves’ Trinkets or Currency. The other best places are Blueprint: Tunnels and Blueprint: Repository. Its Heist target: Unusual Gems. Unusual Gems are Alternate Quality Gems. There are chances to drop Prime Regrading Lens or Secondary Regrading Lens in these two places as well.
Doryani’s Epiphany is a divination card. A set of three can be exchanged for a Regrading Lens.

It is only worked on Active skill gems. If the skill gem has more than 2 alternate quality, the result is random.
The price of the Prime Regrading Lens is about 58 Chaos Orbs. The price is so expensive and the result is random. In fact, there are three alternate quality versions: Anomalous, Divergent, Phantasmal. In most cases, you can buy your desired quality gems from other players directly.
