There are a few cases which are not straight forward such as all minion skills. They say they are spells but are not affected by spell damage. Why aren't minions affected by spell damage when they have the spell tag?
Minions do not "have the spell tag". Minion spell gems have the spell tag, because they are spells. They deal no damage.
There is a fundamental difference between your Raise Zombie spell, which is a spell cast by you that creates a minion and no damage (but would be affected by spell damage modifiers if it did), and the Zombie's Melee attack (which is an attack, not a spell, not used by you, that deals attack damage because it's an attack).
The damage of the Zombie's melee attack is not the damage of the Raise Zombie spell, and is pretty much entirely unrelated to it.
If I have Minion Instability and my Animated Guardian reaches low life, do his exploding item's stats add to the damage of the explosion? For example: if Animated Guardian is wearing "Starkonja's Head" does "50% reduced Damage when on Low Life" apply to the damage of the Minion Instability explosion?
Currently no because this is bugged - the stats are not able to update before the explosion happens. This has been fixed locally for a future patch.
If the Mantra of Flames Jewel's Spell Damage increase per Buff is applied to 3 items (Law of the Wilds, Arakaali's Fang and The Black Cane) is it applied as 1 stack per minion?
Each of those items involves you receiving one buff per minion, yes.
What level are totems/minions/etc? I know it's based on gem level, but is there a table or something to consult? Like gem level 21 = level 75 totem, or something like that?
In general, they have a level matching the level requirement of the skill or support effect that creates them.
Was the lack of a curse tag for Bane and its interaction with Vixen's Entrapment intentional?
Bane is fundamentally not a curse (and never has been). Previously, it was still considered a curse skill (had the tag) because tags are used to indicate what mechanics a skill interacts with, not just what they are. This is also why Dark Pact or the Offerings are Minion skills, despite not creating minions. This caused it to have an unintended interaction with Vixen's Entrapment, which affects socketed curse skills because it was intended to interact with actual curses, but the same tag was used for both cases. This conflicting meaning of skills having the curse tag also caused other issues with designing content, and thus is something we wanted to fix when we had a good solution. When curses were reworked, we had to distinguish between hexes and marks, which meant we now had more tags to work with and could be more specific about what each one meant - limiting the curse tag only to the skills that actually are curses, rather than just interacting with them, and applying the hex/mark tags for skills that interact with hex curses or mark curses as a mechanic. It's quite possible something similar will have to eventually happen with the minion tag to separate skills tagged as creating minions from those tagged as interacting with minions in some form.
How does the Maw of Mischief Helmet skill "Death Wish" prioritize the minions that will be chosen first to explode? Making it choose minions based on proximity of the location of the mouse would make sense, but it seems to be almost random.
The only prioritisation it has is that it cannot affect immortal minions or minions already affected by the skill. Beyond that it is random.
How does Reaper minion decide which of your other minions it will eat when it decides that it's hungry? Is it random or is there a priority of some kind?
The Reaper does not separately decide that it's hungry and then chose a minion. Each time it picks an action, it evaluates all possible actions and picks one. For each of your other minions it can see, there are two possible actions involving eating that minion which will be weighed against other possible actions (similarly for each enemy, there are actions that could attack that enemy, and there are several more possible actions for attacking which are limited only to its current aggro target if it has one).
Eating a minion to heal has a high weighting, but is ignored if the reaper has higher than 75% life or already has the buff from eating a minion.
Eating a minion just for the buff has a lower weighting, but is ignored if the reaper already has the buff or there are no enemies around that minion (so it won't move away from the enemies to eat a far-away minion).
There are no variations in weighting based on minion type, etc, but the Reaper cannot eat "immortal" minions which can't be damaged, such as skitterbots, as they're invalid targets for the eating action, and cannot.eat untargetable minions like raging spirits because they can't be targeted.
Can it eat phantasm minions?
Yes.
Also, are phantasm from summon phantasm support affected by other support gems?
Yes.
Why does skill effect duration increase the duration of some minion skills such as offerings and not other minion skills like skeletons?
This is incorrect. The duration of Skeletons is affected by modifiers to skill effect duration.
Does the modifier given by "Master Toxicist" and Darkscorn "poisons created by you have a x% chance to do x% more damage" apply to poisons applied by skills used by minions, totems and mirages?
The actual modifiers in question are "Poisons you inflict during any Flask Effect have 20% chance to deal 100% more Damage" and "20% chance for Poisons inflicted with this Weapon to deal 300% more Damage". Your minions inflict their own poisons with their own skills (and weapons), so cannot benefit from these.
Totems, Mirages, Traps, and Mines all use your skills, which are affected by your stats, and for attacks, use your weapons as the base. These will be affected.
