List of D2R Achievements Guide.

This trophy required you to complete act 3 and kill the primeval Mephisto and entire the portal to The Pandemonium Fortress. The questline in this act is pretty linear.

You will start out in town and Cain will send you on a journey to find Khalims lost organs to help you enter the dark temple:

1. The first organ (the eye) is located in the spider forest. This is located in a spidery area of the forest complete with spider webs and wrapped-up enemies the spiders have been feasting off of. You will find an entrance called the “spider cavern” inside here is a set of winding webbed-up walls and somewhere on the left side of the area is a unique group of spiders you must kill and then loot the chest and grab Khalims eye to continue on.
2. The second organ (the brain) is located in the flayer dungeon. This place is located in the Flayer Jungle. The beginning part of Act 3 is a huge long corridor of forested trees and rivers with crossing logs you must make your way on and down to continue through the area. Once you reach the Flayer Jungle you can start looking for an area with an entrance called the Flayer dungeon. On the 3rd floor of the flayer dungeon, there is a small maze you must make your way through to encounter Witchdoctor Endugu he is guarding the golden chest that contains Khalims Brain. Kill him and loot the chest for your 2nd piece.
3. The third and final organ (the heart) is located in the kurast bazaar area in an underground place called “The sewers” this place is rather big, but make your way around the area in search of an outcrop with a lever to pull which will lower the ground in front of you and allow you to decent to grab the final golden chest and Khalims heart.

With all pieces in hand and your Horadric cube at the ready make your way past upper kurast into the Travincal. Kill the corrupted council there and the first unique one you slay will drop Khalims flail. Kill the rest of them and clear the area and step foot into the temple next to the glowing red orb. Use your cube to transmute the flail and all 3 organs into Khalims will. Equip the new flail and use it to break the orbs curse and open the temple stair to the “Durance of Hate” make your way to the 3rd floor and kill Mephisto and grab his soulstone and enter the red portal to hell leading to act 4.
