League mechanics: Incursion. While in an incursion, the player can open passages and kill architects to change or upgrade the room, which affects the Temple's layout in the future. After 11 incursions, the Temple in present time can be raided for loot and special altars which modify items.

Vials are currency items used to upgrade certain unique items to their more powerful versions. They are used by placing one in the Altar of Sacrifice in Apex of Ascension (a level 3 room that upgraded from Sacrificial Chamber and Hall of Offerings) along with the required unique item. Vials can only be obtained from The Vaal Omnitect.

Altar of Sacrifice (level 3 room) can be found in Apex of Ascension (level 3). It allows you to sacrifice a unique item for another random unique item of the same item class, including items not ordinarily obtainable.

Table of Sacrifice (level 1 & 2 rooms) can be found in Sacrificial Chamber (level 1 room), Hall of Offerings (level 2).

• Sacrificial Chamber is a level 1 incursion room, it can be upgraded to Hall of Offerings. The room contains a Table of Sacrifice which allows you to sacrifice a unique item.
• Hall of Offerings is a tier 2 incursion room. It upgrades from Sacrificial Chamber and to Apex of Ascension. The room contains an Table of Sacrifice which allows the player to sacrifice a unique item for another random unique item of the same item class.

How to use Vial of Sacrifice?

Sacrifice this item on the Altar of Sacrifice along with Sacrificial Heart to transform it.

A Vial is a currency item used to upgrade unique items. It is used by placing it in the Altar of Sacrifice in a Level 3 Sacrificial Chamber (Apex of Ascension) with the required unique item.

Apex of Ascension

Apex of Ascension is one of the rooms in the ancient Vaal Temple of Atzoatl that can be obtained by upgrading the Hall of Offerings.

The room contains an Altar of Sacrifice which provides the player with one of two different functions:

• Sacrifice a unique item for another random unique item of the same item class, including items not ordinarily obtainable.
• Transforms unique items found exclusively in the Temple of Atzoatl.

The altar may only be used once per temple.

How to get Vial of Sacrifice?

It can only be obtained from The Vaal Omnitect. The Vaal Omnitect is the final boss of The Temple of Atzoatl.

The endboss of the temple is the Vaal Omnitect. It is located in the top chamber, the final temple room.

This room is accessible if you create a route to it during the incursions. You can create a route by killing the mobs until they drop a Stone of Passage, then going to the door you want to open with the Stone of Passage and and opening it. Alternatively, if you have access to the Explosives Room or a higher tier version during the temple instance, you can use the charges found inside it to open a path to the central chamber.

The Vaal Omnitect drops exclusive unique items and corrupted Rare items with an Incursion mod.

The drop level of Vial of Sacrifice is 68. I think the drop rate of Vial of Sacrifice is random. The Vial of Sacrifice seems to be very rare or only from high level temples.
