Andariel, the “Maiden of Anguish”, is one of the Lesser Evils in D2R. It is the final boss of Act I.
She can be found on the lowest level of the Catacombs beneath the Monastery outside of the Rogue Encampment, the starting point of Diablo II. Killing her is the final objective of the Sisters to the Slaughter quest, as well as Act I.
In the Pandemonium Event, players have to track down Lilith and kill her in order to get the ingredients necessary to open a portal to Chaos Tristram. This is not a canonical event and Lilith simply received Andariel’s model because there was no other fitting monster model to use in the game.
The chamber Andariel is in has some monsters in it, notably Dark One, Afflicted and Ghouls. The first room should be cleared before fighting Andariel, who can then be pulled back there from the second room without having to fight many other monsters along with her.
Andariel has three attacks, one melee attack and two ranged poison attacks. Compared to the other bosses she is not very deadly, though new characters may have trouble handling her poison damage if their gear and/or level is not adequately good. This can easily be offset if the player buys Antidote Potions from Akara in the Rogue Encampment however, which will provide a temporary +75 in Poison Resistance.
When Andariel dies, a permanent Town Portal opens up.

Ancient Horadric texts record that Andariel and the other Lesser Evils once overthrew the three Prime Evils – Diablo, Mephisto and Baal – banishing them from Hell to our world.
With this betrayal weighing against her, Andariel’s presence in Sanctuary can only mean that the forces of Hell are once again aligned behind Diablo and his Brothers.
While it is clear that the Maiden of Anguish is acting on behalf of Diablo to prevent anyone from following him eastward, it is also clear that she must be destroyed if the Lord of Terror is to be stopped.
