With the Insatiable Appetite Atlas keystone passive, the Eater of Worlds is fought in Area Level 85 and greatly increases in difficulty. The fight with the Eater will have the following changes:
Area has the mod Monsters take 70% less damage
Eater’s default tentacle attack now attacks in a small ring, dealing overlapping damage.
Tentacle Slams are wider and have different patterns (long wall, 6 sided star).
The Doom Phase always spawns the maximum amount of activatable spheres. Tentacle Slams will occur during the charging phase.
Eater’s Lightning Beam now tracks the player and repeats two additional times.
Sphere Slams have a much bigger area of effect.
Uber Eater has a 25% chance to drop an additional Exceptional Eldritch Ichor, and can also drop a Forbidden Flesh with a hidden Ascendancy Notable skill.
3.22.1d Patch Notes
Damage dealt by the small tentacles in The Eater of Worlds boss fight are now considered to be from The Eater of Worlds, rather than the tentacles. Due to this, Avenging Flame will now target the Eater of Worlds when hit by a tentacle, and the damage dealt by the tentacles will be affected by Invitation modifiers such as Unique Boss Damage.

Basic attack – tentacles are spawned under the player.
Sphere Slam – “Hunger!” – glides to the player’s location and channels a large sphere around itself for ~2 seconds, before delivering a slam around itself. The boss often repeats the spell one more time. Very high damage.
Tentacles Slam – “Writhing mass!” – after a delay, summons a set of tentacles that slam in a direction. Medium damage.
Lightning Beam – “Perish!” – channels for ~1 second, then fires a Shocking beam. Somewhat easily sidestepped. High damage.
Doom Phase – “Inescapable doom!” – the boss teleports to the center of the arena and begins to channel, while tracking the player with Doom Laser with a steadily growing AoE from above. Doom Laser applies a debuff that increases damage taken (Inescapable Doom). Purple spheres are spawned around the arena by Maven. The player has to stand in the circle around each sphere for a few moments to activate them (the sphere begins to glow distinctly). If the player fails to activate all spheres in ~20 seconds, extreme damage is dealt to everything in the arena.
