Random Beyond Encounters:

Beyond Demons are interdimensional demons from parallel realities that attempt to enter the player’s world. The Nightmare realm of the original Beyond demons has been usurped by the Scourge. When in a Beyond-enabled instance, there is a 15% chance that when you kill a monster a small red orb will appear on the ground. If four of these appear close to each other (roughly one screen), they open portals that spawn Beyond demons. Once a substantial number of Beyond demons have spawned in the instance, each pack spawned grants a stacking chance for the next pack in that area to be replaced by a boss. Only one Beyond boss can spawn per instance. Once a boss has spawned, Beyond portals will no longer spawn in that instance.
Demons are divided into three factions, and only one faction will spawn in any single instance. These factions are the Pale Scourge, the Flesh Scourge, and the Demonic Scourge.
Beyond is not a core game mechanic and cannot naturally spawn; however, they can spawn in maps altered by Kirac’s map device mod and are available as a map mod: Otherworldly. Additionally, they can spawn in maps when using a sextant mod that adds Beyond monsters and have a chance to be spawned in maps when using certain Atlas Passive Skill Tree nodes. Certain nodes in Delve also contain Beyond encounters.
These bosses are referred to as the three Demon Lords by Sister Divinia. Both Divinia and the Last to Die claim that Beidat (and other Pale monsters) are blind and appear capable of communication with humans, and feed off of human fear. K’tash appears to be more of a hivemind for the Demonic faction, whereas Ghorr and the Flesh faction appear to be purely driven by the desire to eat.
